Profession of intention: I profess the Christian faith expressed in the Nicene-Constantinopolitan Creed and sincerely seek to live within the doctrine, sacramental life, discipline, and mission of the United Roman-Ruthenian Church. I recognize the canonical authority of the Roman-Ruthenian Pope and the lawful hierarchy of the Church in matters of ecclesiastical faith, worship, discipline, and governance, consistent with Christian conscience and applicable civil law.

I will conduct myself honestly and charitably and will not knowingly misrepresent or bring scandal upon the Church. *

I will not claim to represent the Church, exercise ministry, solicit funds in its name, or use its identity publicly without appropriate authorization. *

I understand that membership creates no entitlement to financial assistance, employment, housing, education, travel, ecclesiastical office, ordination, title, honour, admission to an order, or other material support. *

I certify that the information supplied is true and complete to the best of my knowledge, and understand that material misrepresentation may result in rejection or termination of membership. *
